AI was instructed to destroy fuel tanks. It took out a gas station with above ground fuel tanks as well as a few civilians. 

Most of this is conjecture, but with a strong basis in fact and some evidence. 

An autonomous AI is said to have killed three civilians: Is this the first documented case in history?
